In the work I retired from I was involved for a long time with institutional quality assurance.  I learned early on that problems in delivery of services, while sometimes the result of carelessness or some other individual issue are usually the result of a systemic issue.  Fixing those usually results in much greater reward for the same effort, and avoids future problems.  There is often push back on making system changes.  What is needed to address our healthcare delivery system is a complete system overhaul, and that will only come when those who resist it see that it is in their best interest to get on board.
